FAQ

How do I initiate PT for a resident?

A: It is important to have consent of the patient or DPA prior to initiating our services.  Once obtained, you may fill out the form and we will schedule an appointment.  Through California direct access, an individual can participate in a PT evaluation without the need for a doctor’s prescription.  After the evaluation is scheduled and performed, the plan of care will then be sent to the  physician and follow up visits can be made once it is signed.

Are the therapists experienced?

A: YES! Most of our therapists have over 10 years of experience.  They are board licensed, CPR certified, have a strong geriatric background, and have their own individual liability insurance.

Is this service free for facilities?

A: YES!  There is no obligation.  We ask that you consider us if family or residents are in need of the service.  We would also ask to put brochures in high traffic areas or the front desk.  We can cater our service to the needs of the facility.

How does a resident or family member participate in “other services” you provide?

A: Simply fill out the form below. You can also contact us by phone or email with the information at the bottom of the page.
